NEW PATIENTS
Thank you for choosing Biological Psychiatry Center, P.C. We appreciate every referral and call made to our office. We want to ensure that our office is able to provide the care you need, for that reason all new patients are accepted on a case by case basis. You must call our office to complete our Intake process. Once your case has been approved, a member of our office staff will call asking you to complete our New Patient Forms. Please take a moment to look over our policies and what is expected before your first appointment should your case be accepted by our psychiatrists or therapists.
TELEHEALTH APPOINTMENTS:

Appointments for the doctors must be confirmed by 6:00 pm the evening before the appointment. Any appointments not confirmed will be offered to patients on our waiting list. Please note our office has a voice mail system available 24 hours a day, 7 days per week for our clients to leave confirmation or cancellation notice. Call 586-773-6020 ext 0 and leave a message.

Print, complete and sign New Patient Paperwork. Provide front and back of insurance cards and ID ( Drivers License, State ID or Passport) by email to forms@bpcpc.com. All necessary documents can be dropped off in our building during normal business hours. Call our office to coordinate.. (For your convenience, we are working on getting an e-mail address for all documents to be sent via e-mail.)

Complete the following Telehealth Consent Form (please click to access form) giving us authorization to provide services utilizing Telehealth.

Email front and back of all insurance cards and ID (Drives License, State ID, Passport). ID for minors must be provided, if applicable. (School ID) to forms@bpcpc.com.

If appointment is for a Minor, a parent or legal guardian (no exceptions) must be present at the time of the appointment. In addition, initial paperwork requires signatures of both parents or all legal guardians.

Have your ID available at the time of the appointment for verification of identity. (if minor, please have guardian and patient ID available.)

Contact your insurance carrier, if applicable, to inquire about your Outpatient Mental Health benefits; in many cases, these differ significantly from medical office co-payments or deductibles.

If you are taking prescribed medications:
Our doctors will evaluate you and determine which medications they feel is best. There is no guarantee that you will be prescribed any current or former psychiatric medications prescribed by previous mental health providers or hospitals. If you have ever had an addiction or any form of substance abuse, please know that our doctors will not prescribe any addictive medications.
Medications may not be prescribed on your initial evaluations. Please bring Blood Work, Physicals, EKG's, MRI's or any other records that have been performed within the last 12months.
